You've heard me say it time and again - You have to have a permisison form! Well, I've done up one for the entire year so that your parents/guardians can sign it and you won't have to worry about it until next January.

You can downlad the document from this link and print it off if you haven't got one from church yet. Make sure that you fill it in and give it to me so that you can go to events outside of the church.
